The mikmag theme does not have a shortcode for this. Frankly, I wouldn’t know a plugin to suggest you but there’s got to be quite a lot of them, there must be some good ones to be found.

Mikmag is Gravity forms ready but your more than welcome to use any plugin out there.

Probably the two best Contact Forms availabel are:
[Free] Contact Form 7 – http://wordpress.org/extend/plugins/contact-form-7/
[Premium] Gravity Forms – http://www.gravityforms.com/
If you just want a straight Contact Form without too many bells and whistles, then go for Contact Form 7.
If you want to have a Contact Form with various options and potentially some other type of forms, etc., then Gravity Forms is the way to go.
I’ve used Gravity zForms on a number of sites and would highly recommend it.
