I figured this out.  My .htaccess file was set to:

DirectoryIndex index.htm index.php

This is because index.htm is still the old site that’s running while I upgrade to a new design.  When I make it: DirectoryIndex index.php then it works.

But I still can’t get the slider added to the website because the upload form still opens like it has the wrong z-index setting, and it’s loading behind the main frame (Mik Mag control Panel).
