Have you properly added a form shortcode of some kind? We used Gravityform in the preview website.

I’m unsure what you mean here. Could you be a little more specific about what icons you mean and how you have been creating them?

The icons I mean, are those who are in the main menu, I send you a picture so you can see what I mean.
